1.   ACCEPTANCE OF RISK & MEMBER OBLIGATIONS

a)   Member agrees to strictly abide by Shuttlesport’s Non-Contact Safety Protocols (which can be
found on Shuttlesport’s website and club lobby area) in order to limit the spread of COVID-19. This
includes all provincial public health policies set on physical distancing and proper respiratory
etiquette.

b)   The Member is under an obligation to notify the Club if they observe any Member(s) not strictly
abiding by these new policies set in place by Shuttlesport.

c)   Member agrees that after coming to Shuttlesport, should they experience ANY symptoms of
COVID-19, they will notify Shuttlesport immediately by email to badminton@shuttlesport.com
(available 7 days/week, 24 hours).

d)   The Member will not attend or use any Shuttlesport amenities & immediately self-isolate:

i.    if they are experiencing ANY symptoms of COVID-19;
ii.   if they have been in immediate contact with anyone who is experiencing any symptoms of
COVID-19;
iii.  if they have or been in contact with someone who has travelled outside of Canada in the
past 14 days;

e) Member understands and agrees that Shuttlesport may provide your contact information to public
health officials to assist with contact tracing to limit the spread of COVID-19.

2.   ACCEPTANCE OF OBLIGATIONS

a)   The Member accepts their obligations under this “Agreement” and acknowledges that a breach of
these obligations on their part will result in immediate suspension.

b)   The Member hereby releases the Club, its Directors, Officers, and Staff from any and all claims that
the Member has or may have in the future for:

i.    any loss or damage the Member may suffer due to contracting COVID-19, including sickness
or death, as a result of attending or using any or all amenities; and
ii.   any suspension or other discipline imposed on the Member by Shuttlesport Badminton
Academy resulting from the Member’s breach of their obligations under this “Agreement”.
